How It Works

    

  1. Initial Consultation: Schedule a complimentary      consultation to discuss your health goals and needs.
  2. Enrollment & Welcome: Our team guides you through a      simple enrollment process and sets up your personalized care plan.
  3. Technology Setup: We deliver and configure your communication and monitoring devices, training you and/or your family/caregivers on how to use.
  4. Visits & Other Services: Enjoy regular tele-health check-ins, in-home visits, and 24/7 access to your care team. We provide flexible service delivery to fit your needs and lifestyle.
  5. After Visit Care: We create and send your Medication Action Plan (MAP), updated Medication List (ML), and other recommendations to the appropriate health team member (yourself included). The MAP is a personalized medication roadmap to wellness through proper medication use, while the care plan  details other recommendations for your PCP health team members (e.g., follow-up visits, referrals to specialized services such as nutritionists, health coaches, etc.) recommended labs, medication access assistance,  etc.) to optimize health goals and outcomes.
  6. Family & Caregiver Involvement: We keep your authorized loved ones informed and engaged, providing education, auxiliary support and updates as needed.
  7. Ongoing Monitoring & Outcomes Tracking: We keep a close eye on you via regular follow-ups, wellness and adherence checks, education, RPM, lab, symptom tracking, etc. 
  8. Continuous Optimization & Support: Your care plan will evolve with your needs and will be updated accordingly so you always receive the best possible care.

  


FAQs (Frequently Asked Questions)


Q: What are concierge pharmacy (medication) services?


A: Concierge pharmacy is a service-oriented model that places the patient and their health at the center of care. Unlike traditional brick-and-mortar pharmacies that focus on dispensing drug products with very little ongoing medication management input, concierge pharmacy practices offer personalized, in-depth support and medication management services tailored to each individual’s unique needs. Pharmacists work closely with patients and their doctors to provide tailored healthcare plans, comprehensive medication reviews, adherence interventions, and ongoing education. This model ensures that patients have clinical advocates who understands their medical and related situations and who help them get the most out of their medications.


Q: What are Remote Patient Monitoring and Transitions of Care?


A: Remote Patient Monitoring (RPM): RPM uses digital devices to monitor a patient’s vital signs and health data from home. This information is transmitted electronically to healthcare providers, who can track the patient’s condition and intervene if needed. RPM helps manage chronic diseases, supports post-discharge care, and reduces hospital readmissions.


A: Transitions of Care (TOC):
This refers to the coordination and continuity of healthcare as patients move between settings - such as from hospital to home or between different providers. Effective transitions of care ensure that patients receive the right care at the right time, reducing medication errors and improving outcomes.


Q: Who can benefit from these services?


A: Patients with chronic conditions, complex medication regimens, or those at risk for hospital readmission can benefit greatly from concierge pharmacy, remote patient monitoring, and transitions of care services. These are especially helpful for older adults, patients transitioning from hospital to home, and individuals needing ongoing support for medication adherence and disease management.


Q: How does billing work?


A: We charge membership fees, enhanced service fees (for Premium services), and fee for ad hoc services using a value-based payment model. While we can bill for certain servixces such as chronic condition Management and RPM, you may have to to pay out-of-pocket, utilize your long-term care assurance (if appropriate) or use your health savings account (HSA) for personalized care not covered/billable by your insurance. Kindly check with your health plan to ensure reimbursement FIRST before scheduling an appointment other than the initial FREE consultation.


Q: What are the costs?


Q: How do you coordinate with my doctor?


A: EPC works closely with your primary care physician and other healthcare providers. We communicate regularly to share information, coordinate medication management, and ensure continuity of care. This collaboration helps prevent gaps in treatment, supports smooth transitions between care settings, prevents medication omissions and errors, and ensures all providers are aligned with your treatment plan.
