About half of the U.S. population uses prescription medicines on a chronic basis, including an estimated 85% of seniors. In 2015, the U.S. reported that prescription drugs had caused over 81 million adverse reactions and resulted in 2.7 million hospitalizations. This epidemic of medication harm makes prescription drugs the fourth leading cause of death in the United States, and these numbers continue to escalate. Medication-related hospital admissions were as high as 41.3% by 2017, with a third being preventable and a third potentially preventable.

Pharmacists are highly trained medication use experts whose scope of practice extends beyond preparing and dispensing drugs. They provide services in various healthcare settings, such as hospitals, clinics, drug store chains, facilities, or at home. Many scientific studies have demonstrated they are pivotal in reducing adverse drug reactions and related harms (e.g., hospitalizations and death), and curtail medication-related and overall healthcare costs appreciably when integrated within patient care teams. Clinical Pharmacists are thus one of the most instrumental but under-utilized members of patients' healthcare teams regarding medication-related outcomes. Our Services

Concierge Model: Our services are comprised of any or a combination of the targeted solutions listed below, tailored to what is most appropriate for your needs at any given time. 

 We provide round-the-clock access and highly personalized care with our Clinical Pharmacist Champions. These pharmacists asdvocate on your behalf for your best health and desired goals in every regard, and are accessible 24/7 to champion your medication-related needs. These include emergencies and hospitalizations, in-home medication assessments and service visits, liaising with your providers and other health team members to make medication recommendations, monitoring your progress, coordinating your care, and anything else necessary and within our scope of practice to optimize your treatment outcomes.

Specific Services Include:

Disease (or Chronic Condition, i.e. CCM) Management: A collaborative and targeted combination of the services below aimed to improve and maximize the health outcomes associated with specific disease states. Some examples are diabetes, hypertension, heart and vascular diseases, thyroid disease, mental health disorders, etc.

High Risk Medication Assessment: A targeted assessment of medications considered to be high risk in the elderly and of their potential, actual or perceived adverse effects.

Medication Check-Up: A quick (15 to 30 minute) touch-point with a pharmacist (via Telehealth, phone or secure, real-time messaging) to assess your medications for appropriateness based on the information provided.

Medication Review: Similar to a Check-Up, this includes an assessment of your medications in view of your stated medical conditions to ensure that they are appropriate. It may be targeted (focusing on a particular medication, type of medications or disease state e.g., diabetes medications) or comprehensive (reviewing all your listed medications).

Medication Therapy Management: Combines a Medication Review & Reconciliation such that your medications & medical conditions are examined to see if the combinations are safe, effective and successful in helping you achieve your health goals & improve your status. We generate and send an updated Medication List, Medication Action Plan (your to-do list), and Pharmacotherapy Recommendations (suggestions to your doctor or other provider) after each visit and perform other follow-up functions as necessary to ensure your optimal health.

Medication Education: We have a variety of educational tools and formats to teach and train individuals and groups on a wide array of disease management & medication-related skills & subjects.

Remote Patient Monitoring: Certain disease outcomes (e.g., diabetes, hypertension, heart failure) can be optimized by connecting seniors with devices that monitor and transmit key information to their care team (i.e., doctors, EPC pharmacists, and other providers), all from the comfort of their homes. The same technology can be applied to medication administration and other tracking parameters. This innovative approach supports 24/7 care,  direct communication between patients/caregivers and providers, and quick interventions when needed.
